1. ZionsHope.com (ZH) will maintain a banner for ____________________________ (herein known as Advertiser) on the ZH site for the sum of ______________________ per banner selected below per the termed period of __6 months or __12 months.
    
2. The banner will be placed in at the selected four locations:

   ___ Rotating 468 x 60 pixel At the top of the ZH home page;
   ___ 100 x 60 pixel On the ZH home page;
   ___ 468 x 60 pixel At the top of the ZH sub-page;
   ___ 100 x 60 pixel On the ZH sub-page;

3. The banner will be placed online within five (5) business days of receipt of both the banner image and payment from Advertiser, and the termed placement period will begin the day after the banner goes online. Advertiser will be notified with 24 hours of their banner going online;
4. The banner will be supplied to ZH by any of the following methods:
   1. As an e-mail attachment mailed to advertise@zionshope.com
   2. As a specified URL if the banner is already online at another web address;
   3. Via standard mail on a 3-1/2 IBM-compatible diskette.
5. Banner supplied by Advertiser will:
   1. Be either GIF or JPEG format;
   2. Not exceed 468X60 pixels in size (unless otherwise agreed upon);
   3. Will not contain false or misleading information, or content inappropriate for the visitors of ZH. The banners preferred must be Health or medical related.
6. Advertiser understands that they will have no editorial input into the contents of the ZH web site.
7. ZH may refuse any banner before it is placed online for any of the above reasons, or for any other reason ZH deems sufficient. If Advertiser's banner is refused, all fees paid for the pending period will be returned in full to Advertiser within five (5) business days;
8. ZH will make available to Advertiser unedited ZH traffic statistics in HTML format at least once every week. Statistics will cover the current month to date, and will be placed at a specified URL by Friday afternoon of each week.
9. Unless written notice is given five (5) business days before the current advertising period expires, Advertiser's banner will be automatically renewed at the end of the termed period unless other arrangements have been made in writing;
10. Payment for subsequent termed placement will be due by or on the first day of the next period.
11. Payment may be made:

    By check drawn on a U.S. bank and made payable to Zion's Hope Global Medical. Checks are to be mailed to:

12. ZH reserves the right to remove Advertiser's banner from site rotation if payment is not made within the timeframe or in the manner specified above;
13. Advertiser may request that their banner be removed from the ZH site at any time. The banner will be removed from the site within 24 hours of receipt of a written request;
14. ZH reserves the right to remove Advertiser's banner from the site for any reason. ZH will reimburse Advertiser on a pro-rated schedule for the remainder of the termed period.
